Please read the following Trio Regional Program Contract Statement and sign below if you agree to the terms and conditions if accepted into the program.
I agree to participate in the 2021 Morehouse College Upward Bound Math/Science Southeastern Regional Program. I understand that the program dates are June 4, 2021 to July 9, 2021. I have no other obligations that will interfere with my full participation in the program for these dates. By signing this contract, I know that I am confined to participation in the program upon acceptance into the program. If an emergency develops, I will contact the program manager and a decision will be made regarding program next steps. I understand that my participation for the duration of the program is also dependent upon my behavior during the program. I understand that if my behavior is unacceptable that I will be dismissed from the program. I understand if I am dismissed, my transportation away from the program must be taken care of by my parent(s)/guardian(s). Upon completion of the program, I agree to submit copies of my future transcripts and acceptance letters to post-secondary schools to the program manager and I agree, if possible, to participate in TRIO Alumni follow-up activities as notified by the program.
