I agree to participate in the 2021 Morehouse College Greater Orlando Upward Bound Math/Science Program. I understand that the School year program is Fall 2021 to Spring 2022. The 2021 Summer Math/Science Program dates are May 31, 2021 - July 2, 2021. I have no other obligations that will interfere with my full participation in the program. By signing this contract, I know that I am confined to participation in the Math/Science Program. If an emergency develops, I will contact the Program Manager and a decision will be made. I understand that my participation is dependent upon my behavior. I understand that if my behavior is unacceptable, that I will be dismissed from the program. I understand if I am dismissed, my transportation must be taken care of by my parent/guardian. Upon completion of the program, I agree to submit copies of my transcript, copies of acceptance letters to post-secondary schools, and agree to participate in alumni follow-up.
